feat: spawn composite type-10 npcs with life packet

- Add MsgNpcInfoEx.CreateComposite for ordinary NPCs that need HP

- Route type-10 NPCs with life through 1109 in Vision.Add and SendSpawn

- Leave SOB, booth, and event NPCs on their existing spawn packets
